    Paste it into a word document, fill in your info and print.

    Get your AO to photocopy your permit, write 'certified copy' and sign it.

    Post the 4 documents to:

    International Licensing Department
    Attn: Sandy

    Montezuma, IA 50171

    I would suggest 2 more things,

    a) Sign and DATE the purchase order. Sandy sent my purchase order back as while I had signed the covering letter I had not signed AND dated the purchase order and the state dept f**kwit rejected it. Anyway my spikes BCG was paid for and shipped yesterday yeah!

    b) Ring/email your AO and just let him/her know you are doing this, just in case you get an awkard asshole in NZ Customs. Shouldn't happen for a trigger but Ive had to deal with some real assholes in Customs over the years who frankly are clueless, arrogant, ignorant pricks who have huge power to do as they please and weild it. I also think its good to build a relationship with your AO, keeping on his good side is worth the 5min call/email IMHO.
