i had one in only last week, it was a 17hmr, had done the self distruct it then had been rebarreled to 22mag, the owner had 2 or 3 packets of ammo thru the new barrel when it discharged partly in battery, 6mm of case in the chamber, the rest exited via the ejection port, taking the extractor with it, the mag was fused into the receiver.
I is the third one I have seen personally, this year.
all 17hmrs are rebarreled as they come in because it is just a matter of time before they let go.

Interested to know if all three where magnum research models, with the chamber pressure block hanging off the bottom of the chamber?
