It's just a spiral bound note book. It has all of the locations in it too I will see if I can work out how to black the locations out of a pic of a couple of pages.
I put in it when, where, range, rifle, projectile. Details of the deer seen and where, and details of the deer I shot (if I did). And who I was with (if anyone) and details of their kill if they got one. I record every trip.

I also keep a blog (on another site) of everything I get with my dog.
