I doubt that further levels of bureaucracy would make a tangible difference in the frequency or outcome in this type of incident. Personally I have no desire to have to pay a stranger to deem me mentally stable on renewal of my licence, two personal referees accomplishes the same thing quite effectively in my opinion.
You can't legislate risk entirely out of any given activity.
